Sports physicals for 2025-2026

​Students who will be in grades 6-12 this school year who want to compete in middle school or high school athletics must have their physicals taken before practice begins. The student needs to have a physical form on record at the school with an actual physical being done every two years. An interim form (no physical required) must be signed by the parents during the in-between year. Parents can choose to have their child do a physical every year. If you do not know which type is needed for next year, you can contact Jim Kocer at 583-2773, or e-mail him at jim.kocer@k12.sd.us. The forms, set by the SDHSAA, have been updated and the parents should take the time to complete them accurately.

​The interim form is one sheet and the physical form is on two sheets (the first sheet should be completed before going to the medical provider doing the physical). If you need an interim or physical form, you can print them by going to the www.sdhsaa.com site and go to forms, then athletics, and then click on physicals exam (the forms are on the last three pages).

Physicals can be taken at the Scotland Medical Clinic (583-2227). If you choose to go elsewhere for your physical, be sure you have the proper forms with you. Students will not be allowed to practice or compete until the physical forms are in. All of the forms should be turned in to Jim Kocer at Scotland High School.
